To help me receive a better understanding of your general health and massage needs, and as per the requirement of my association, it is mandatory that all new clients complete the patient form before your first massage treatment. If you have a specific medical condition, massage therapy may be contraindicated. A referral from your primary care provider may be necessary before service can be provided.
